Azalea Place provides 12 independent living apartments for chronically mentally ill adults and their families. The facility was designed with two 2-bedroom and ten 1-bedroom apartments. The ground floor includes a community room, resident laundry, and manager's office.

Owner: Azalea Place Non-Profit Housing, affiliated with Vancouver Housing Authority.
Funding: U.S. Department of Housing and Urban Development, 811 Program.
